Community Nursing Services to DVA Card Holders
Our primary service is delivering community nursing care to holders of the Veteran Gold Card and Veteran White Card. We work under referral from your GP, nurse practitioner, or hospital to assess your needs and deliver professional nursing care in your home — helping you maintain independence and avoid unnecessary hospitalisation.

Home and Community Needs Assessment
Our Registered Nurses conduct thorough in-home assessments to understand your unique health status, personal care requirements, and living environment. This evidence-based assessment forms the foundation of your personalised care plan, ensuring all services align with your current needs and clinical goals.

Highly Specialised and Complex Care
Our skilled nursing team delivers highly specialised care for complex health conditions, including managing chronic wounds, post-operative recovery, advanced symptom management, and end-of-life/palliative care. All care is delivered in accordance with your nursing care plan by qualified and competent staff.

Medication Management and Administration
Our nurses assist clients with comprehensive medication support — including medication review upon admission, safe storage, proper administration, and ongoing monitoring. All medication management follows strict clinical protocols and requires appropriate medication authority or a signed medication chart from your treating practitioner.

Wound Care Management
Our highly trained nurses provide expert wound care — assessing chronic and complex wounds, formulating and following personalised treatment plans, and applying current evidence-based wound management practices. From post-surgical wound care to management of ongoing skin conditions, we ensure effective and safe healing.

Coordination of Services
We collaborate closely with clients, families, GPs, hospitals, and other health professionals to coordinate care seamlessly. Our nursing staff helps navigate referrals, arrange appropriate services, and ensure your care plan reflects your health, community, and personal goals — all while keeping you informed and in control.

Oracle Ability Services is a registered provider under the Department of Veterans Affairs (DVA) Community Nursing program. If you hold a DVA Veteran Gold Card or White Card, you may be eligible for fully funded community nursing care at home. Contact us or speak with your GP to arrange a referral.

Am I eligible for DVA community nursing services?
You are eligible if you hold a Veteran Gold Card (covers all assessed clinical nursing and personal care needs) or a Veteran White Card (covers care for accepted service-connected injuries or conditions). To confirm eligibility, contact the DVA Provider Enquiry Line on 1800 550 457.
How do I start receiving services?
You will need a valid written referral from your GP, nurse practitioner, treating hospital doctor, or hospital discharge planner. Once we receive your referral, our Registered Nurse will conduct an in-home assessment and develop a personalised care plan with you.
Where are services delivered?
All community nursing services are delivered to you in your own home, face-to-face. Where face-to-face is clinically appropriate but not possible, remote care (by phone or online) may also be arranged.
Do you provide 24/7 support?
Yes. All clients are provided with an emergency contact available 24 hours a day, 7 days a week.
What areas do you service?
